Weight Management Program Membership

Description

Monthly weight management program that focuses on the individual causes of weight gain and creates an individualized plan to assist with healthy weight loss. Our program provides education on lifestyle changes, nutritional support, and medication therapy. Enrollment in the weight management program is $250 and the cost of each monthly appointment varies based on medications prescribed. Lowest monthly membership fee is $75 and increases based on need for oral or injectable medications. This is deposit for the appointment for the service. Deposit is applied to total cost of service. Deposit is nonrefundable.
